According to the National Institutes of Health and the Centers for Disease Control and Prevention, nearly one-third of persons 65 years and older have diabetes. Diabetes Self-Management Program (DSMP) is a proven intervention that can help lessen disease severity and empower people with diabetes to take an active role in managing their health.

In our Reducing Disparities in Diabetes Care: Everyone with Diabetes Counts initiative, the New England QIN-QIO offers evidence-based resources and strategies, aligning with the national Everyone with Diabetes Counts initiative, to improve the health status of people with diabetes and pre-diabetes.
