The New England QIN-QIO connects healthcare partners (e.g., hospitals, physician groups, nursing homes, home care agencies, community-based organizations) and stakeholders to enhance transitions of care for the patients they share.

Working together with the community coalitions, we aim to: improve transitions of care for all patients, reduce unplanned hospital admissions and readmissions, decrease the prevalence of adverse drug events and help combat antibiotic resistant bacteria through outpatient Antibiotic Stewardship.


Sharp Reduction in Avoidable Hospitalizations Among Long-term Care Facility Residents

For long-term care facility residents, avoidable hospitalizations can be dangerous, disruptive, and disorienting. Keeping our most vulnerable citizens healthy when they are residents of long-term care facilities and reducing potentially avoidable hospital stays has been a point of emphasis for the Centers for Medicare & Medicaid Services (CMS).
